Insurance Policy & Assignment of Benefits

Insurance Verification and Your Responsibility

Crown Counseling verifies insurance eligibility as a courtesy; however, verification is not a guarantee of coverage or payment. You are responsible for understanding your own mental health benefits, including your deductible, copay, coinsurance, and out-of-pocket maximum. Any balance not covered by your insurer, including deductibles, copays, coinsurance, and non-covered services, is your responsibility and will be billed directly to you. All copays and known patient balances are due at the time of service.

Changes to Coverage

To avoid claim denials and unexpected balances, you are responsible for notifying Crown Counseling promptly of any changes to your insurance coverage, including new insurance, changes in employment, plan cancellations, or failure to re-certify. Please also disclose all active insurance plans, including any secondary coverage. If your insurance is inactive at the time of a session, you will be responsible for the full session fee.

Assignment of Benefits

By initialing the Assignment of Benefits on the intake form, you assign to Crown Counseling, PC all rights and benefits under your applicable health insurance policy for services rendered. You authorize Crown Counseling to submit claims to your insurer on your behalf, to receive payment directly from your insurance provider, and to release any clinical or billing information necessary for the processing of your claims.
