Consent for Treatment

By engaging in services at Crown Counseling, you are voluntarily consenting to receive mental health treatment, which may include clinical assessment, diagnosis, and psychotherapy, as determined appropriate by your clinician in collaboration with you.

Benefits and Risks of Therapy

Therapy can be a powerful and effective intervention. Many clients experience meaningful reductions in distress, improved relationships, and resolution of specific concerns. However, outcomes cannot be guaranteed. Progress depends on many factors, including the nature of your presenting concerns, your active engagement in the process, and consistency of attendance.

There are also inherent risks to therapy. You may be asked to explore uncomfortable emotions, revisit painful experiences, or confront patterns that are difficult to examine. These experiences are a normal part of the therapeutic process and are not signs that therapy is not working. Your clinician will support you through them.

Your Rights as a Client

You have the right to ask questions about your treatment at any time, to participate in the development of your treatment goals, and to refuse or discontinue any recommended service. You may withdraw from treatment at any time without penalty.

Crown Counseling is not a crisis or emergency service. If you are experiencing a psychiatric emergency, please call 988 (Suicide and Crisis Lifeline), call 911, or go to your nearest emergency room.

Acknowledgment

Clients confirm on the intake form that they have read and understood this information and voluntarily consent to receive mental health services at Crown Counseling.
