Telehealth Consent

Crown Counseling offers telehealth services via secure, HIPAA-compliant video and telephone platforms. Participation in telehealth is voluntary.

Your Responsibilities

When participating in telehealth sessions, you are responsible for ensuring you are in a private location where you cannot be overheard, that you have a reliable internet connection and a device with functioning audio and video capability, and that you are located within the state of New York at the time of your session. Crown Counseling cannot be responsible for technical difficulties arising on your end, including loss of connection or equipment failure.

Limitations and Safety

Telehealth has inherent limitations compared to in-person care. Some clinical presentations may not be appropriate for remote treatment, and your clinician may recommend transitioning to in-person services or a higher level of care if clinically indicated. In the event of a psychiatric emergency during a telehealth session, your clinician may contact emergency services at your location. For this reason, you will be asked to provide a current physical address and emergency contact prior to the initiation of telehealth services.

Acknowledgment

Clients confirm on the intake form that they consent to receive services via telehealth and understand the responsibilities and limitations described above.
